By checking, I agree and acknowledge that the Guest Suite requested is located in my building.

(ex: If you live in UHV 4 - 5520 W Success, you are requesting to reserve the Guest Suite in that building)

I have read and agree to the following Guest Suite Rules: 

1. Persons under eighteen (18) must be accompanied by a parent or Guardian.

2. Absolutely No Smoking allowed in the community room.

3. If equipment or appliances are not working properly please report the problem to the office as soon as

possible.

4. Reservations must be made at least 48 hours in advance, Monday through Friday, and no later 

than 3:00 PM on Fridays.

5. The cleaning fee is included in the nightly rate. 

6. Cancellations need to be made 24 hours before your reservation or will be charged the nightly rate. (Cancellations should be made by email to office@dynamic605.com)

7. Tenant is responsible for any damages and/or missing items in the Guest Suite. 

8. The Guest Suite will be available after noon (12 PM) on the start date reserved, and guests will need to be out at noon (12 PM) on the end date reserved.

The Guest Suites are for your entertainment and enjoyment. Please 

use care and caution when using the facility and operating the equipment. Any damages or missing items incurred while 

using the Guest Suites will be charged to the resident who reserved the room.
